This remote position allows you to work flexibly from the comfort of your home, with consistent support and communication from the team. You will collaborate virtually with colleagues, participate in team meetings, and maintain a seamless workflow, leveraging technology to ensure effective communication and care delivery.

• Conduct thorough assessments of clients to gather relevant information about their circumstances and needs, ensuring an accurate understanding of each individual’s unique situation.
• Provide crisis intervention services promptly, using your skills to de-escalate tense situations and offer immediate emotional support.
• Engage clients in beneficial activities by identifying opportunities for involvement in community resources and services that promote their well-being.
• Collaborate with clients, families, and external agencies to coordinate comprehensive care, making necessary referrals to specialized services that will enhance support.
• Document interactions meticulously, maintaining accurate records and reports to contribute to ongoing evaluation of client progress and care plans.
• Participate actively in professional development through training and supervision, ensuring continuous improvement of skills and knowledge.
• Remain vigilant for signs of child abuse and understand mandatory reporting requirements, ensuring the safety and well-being of vulnerable populations.
• Take part in team meetings to share insights and strategies, fostering a collaborative environment that enhances client care.

Your primary focus will be on clients who are experiencing significant emotional and psychological challenges due to historical trauma, mental health issues, and social instability. Particularly, you will support individuals and families dealing with the multifaceted impacts of hospice and end-of-life experiences, guiding them with empathy and professionalism during difficult transitions.

• A Master’s degree in Social Work, Psychology, or a closely related field.
• Current and active licensure as a Clinical Social Worker (LCSW).
• Exceptional interpersonal skills, with the ability to empathize with clients and remain calm under pressure.
• Strong computer proficiency, including familiarity with Microsoft Word, Excel, PowerPoint, Outlook, and telehealth platforms.
• The capacity to engage in active listening, ensuring that clients feel heard and understood during their sessions.

• Demonstrated experience working with clients affected by neglect, domestic violence, mental health disorders, and substance abuse issues.
• Familiarity with crisis intervention techniques and best practices in a remote work setting.
• Proven ability to develop individualized treatment plans based on thorough client assessments.
• Knowledge of local community resources to facilitate effective referrals.
