This role operates within a remote setting, allowing for flexibility while connecting with clients across various platforms. Despite the remote nature, collaboration with teams and community partners is vital, facilitating a cohesive approach to service delivery in our community health clinic framework. While core working hours generally align with business hours, occasional evenings or weekends may be necessary for events or urgent client needs, fostering a community-embedded approach.

• Conduct thorough and holistic assessments of clients to identify their social, emotional, and psychological needs, ensuring tailored support.
• Deliver evidence-based individual and group counseling sessions, employing therapeutic techniques that promote healing and resilience.
• Design and implement comprehensive service plans that align with client goals and objectives, tracking progress and making adjustments as necessary.
• Actively advocate for clients' rights by navigating systems and resources, ensuring equitable access to supportive services and raising awareness of social justice issues.
• Engage in program development by contributing to evaluation processes, grant writing, and reporting initiatives to enhance service delivery.
• Collaborate effectively with community organizations, local agencies, and volunteers to expand the range and impact of our services.
• Participate in ongoing training, team meetings, and community outreach to stay abreast of best practices in social work and community health.

Your caseload will primarily consist of individuals and families from diverse backgrounds, including those experiencing economic hardship and various mental health challenges. You will play a critical role in addressing issues such as trauma, substance abuse, grief, and family dynamics, utilizing a culturally competent approach to foster trust and openness.

• A Master’s degree in Social Work (MSW) from an accredited institution is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) is essential to practice.
• Proficiency in trauma-informed care principles and familiarity with social justice frameworks is expected.
• Strong communication, empathy, and organizational skills are critical, as is the ability to navigate sensitive topics with clients efficiently.

• Previous experience working in nonprofit organizations or similar community service settings is highly valued.
• Familiarity with community resources and advocacy efforts to support underserved populations is a plus.
• Experience utilizing electronic health records (EHR) systems to document sessions and manage case files effectively.
