About the Role

We're seeking an experienced Community Health Worker to provide care coordination and support services to underserved populations. You'll work with individuals enrolled in enhanced care management programs, helping them navigate healthcare, housing, and social services while addressing barriers like homelessness, chronic illness, and behavioral health challenges.

Key Responsibilities
  • Conduct community outreach and educate people about available healthcare and social services
  • Enroll eligible members into managed health plans
  • Manage a caseload and coordinate referrals from health plans
  • Navigate members through healthcare, housing, behavioral health, and social service systems
  • Conduct home and community visits to assess needs and provide health education
  • Connect members with appropriate medical, housing, behavioral health, and community resources
  • Advocate for individuals facing complex physical, behavioral, or social challenges
  • Use motivational interviewing to encourage positive behavior change and self-advocacy
  • Collaborate with healthcare providers, social workers, health plans, and community organizations
  • Maintain accurate, timely, and confidential documentation
  • Collect and report required data
Required Qualifications
  • Minimum 2 years of professional experience in community health, outreach, care coordination, or related field
  • Demonstrated experience enrolling members into managed health plans
  • Strong communication, cultural sensitivity, advocacy, and documentation skills
  • Ability to conduct home visits and travel locally throughout the assigned county
  • Must live within or near the assigned service area
Preferred Qualifications
  • Bachelor's degree in Social Work, Psychology, Sociology, or related discipline
  • Community Health Worker certification
  • Knowledge of healthcare systems and medical terminology
  • Familiarity with Medi-Cal, Medicare, CalAIM, or similar programs
  • Experience supporting individuals experiencing homelessness, behavioral health conditions, substance use disorders, or complex social needs
