Positions in this function address efforts focused on medical, behavioral, and social determinants of health factors that improve the well-being of individuals, families, and communities.


You'll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges. Ideal candidate will reside in the western part of Washington State.


Primary Responsibilities:

  • Serves as the primary subject matter expert (SME) and point of contact for Social Determinants of Health (SDOH) initiatives across the health plan
  • Ensures compliance with state and national SDOH, health equity, and community engagement requirements
  • Facilitates cross-departmental action planning and implementation of SDOH and health related social needs (HRSN) initiatives
  • Oversees provider education and outreach campaigns related to SDOH and health equity
  • Drives continuous improvement efforts to strengthen SDOH programs, processes, and outcomes
  • Leads and coordinates Community Advisory Councils (CACs) and Member Advisory Councils (MACs)
  • Manages advisory council recruitment, engagement, communications, and meeting operations
  • Manages strategic partnerships with community organizations and external stakeholders
  • Represents the health plan in external collaborations, workgroups, and community-based initiatives
  • Builds and maintains solid relationships with internal and external stakeholders

Required Qualifications:

  • 1 years of experience in local community-based resources
  • Intermediate level of proficiency in MS Word, and Outlook
  • Designated workspace inside the home with access to high-speed internet
  • Access to reliable transportation & valid US driver's license


Preferred Qualifications:

  • Undergraduate degree 
  • Experience working in Managed Care
  • Clinical, case management, or community health worker experience
  • Knowledge of Medicaid and / or Medicare population
  • Knowledge of and experience working with social determinants of health and/or health related social needs
  • Skilled at working across departments and aligning processes
  • Bilingual Spanish and English
  • Proven ability to lead initiatives, manage timelines, and ensure deliverables are completed
  • Proven ability to use data to guide interventions to advance health equity, improve member outcomes, and improve access to SDOH resources
  • Proven ability to work some evenings and weekend hours for community events (if in Washington state)
  • Reside in the western part of Washington State
  • Ability to travel in assigned territory approximately 10% of the time (if in Washington State)

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $24 - $43 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
