Key Responsibilities
  • Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues as part of the healthcare team.
  • Conduct comprehensive patient intakes, focusing on diagnostic and functional evaluations to create tailored treatment plans.
  • Deliver brief, focused interventions for patients seeking mental health support, guiding them towards positive outcomes.
  • Provide recommendations to the primary care team regarding patients' treatment goals and ensure appropriate referrals to specialty mental health facilities when necessary.

Qualifications
