This position is fully remote, allowing you to provide care and support to clients from the comfort of your own home while maintaining a work-life balance. You will collaborate virtually with a multidisciplinary team, including physicians, nurses, and other mental health professionals, to ensure holistic care delivery. Our remote setup is facilitated by advanced telehealth platforms, ensuring seamless communication and service delivery that meets the needs of our clients.

• Conduct thorough client assessments through virtual sessions to establish a comprehensive understanding of their emotional and psychological needs.
• Develop and implement personalized treatment plans tailored to each client's unique situation, focusing on supporting them and their families through mental health challenges during hospice care.
• Provide individual therapy sessions, as well as couples and family therapy, during which you will employ a variety of evidence-based therapeutic modalities suitable for end-of-life scenarios.
• Collaborate closely with other health care professionals to create and maintain integrated care plans, attending virtual team meetings to discuss client progress and outcomes.
• Maintain accurate and timely records within our electronic health record (EHR) system, documenting assessments, treatment plans, and progress notes in compliance with all legal and ethical standards.
• Engage in regular professional development through supervision sessions and training programs to stay current with best practices in mental health care and hospice support.

The ideal candidate will have a strong focus on serving a diverse patient population, including individuals facing terminal illnesses, their families, and caregivers. You will play a crucial role in providing emotional support, facilitating grief counseling, and addressing complex psychological issues that arise during end-of-life transitions.

• A Master’s degree in Social Work from an accredited program is required to ensure a solid foundation in clinical practices.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is essential, ensuring compliance with local regulatory standards.
• A minimum of three years of experience in mental health or outpatient settings, with a specific focus on hospice care preferred.
• A thorough understanding of evidence-based therapeutic modalities and treatment approaches as they relate to grief and loss.
• Exceptional interpersonal, communication, and assessment skills are necessary for building rapport with clients and their families in sensitive situations.

• Experience working with terminally ill patients and understanding the unique challenges they, along with their families and caregivers, face.
• Familiarity with telehealth technologies and best practices to ensure effective virtual therapy sessions.
• Proven ability to work collaboratively in a team-oriented environment while maintaining a strong sense of independence.
