Job Summary
Our client is seeking a Clinical Pharmacist to join their Clinical Care team. In this role, you will leverage cutting-edge software to optimize patient outcomes and enhance patient care. You will engage with patients and providers, provide expert medication consultations, and collaborate to improve software platforms while staying updated on pharmaceutical developments.
Responsibilities / Job Duties
  • Engage patients over the phone to deliver clinical services, including Medication Therapy Management (MTM) services, medication adherence counseling, and clinical program education.
  • Review and document patient data within proprietary software.
  • Coordinate care with clinics, providers, pharmacies, and patients.
  • Develop clinical intervention strategies to optimize medication utilization and meet healthcare goals.
  • Manage patient, provider, and pharmacy interactions, maintaining high productivity and service quality.
  • Meet individual and team performance metrics.
  • Adapt and rotate across clinical projects based on business needs.
  • Identify opportunities to improve workflows and software platforms.
  • Provide daily sign-out and maintain clear handoff communication for continuity of care.
  • Coach and support Clinical Care team members.

Minimum Education and Experience Qualification Requirements
Education
  • Doctor of Pharmacy (PharmD) from an accredited college or university.
  • Current, unrestricted Pharmacist license in the State of Utah.

Qualifications
  • Deep understanding of quality measures and medication policies.
  • Comprehensive knowledge of chronic disease states and pharmacology.
  • Strong aptitude for direct patient engagement and communication.
  • Demonstrated reliability and strong attention to documentation accuracy.
  • Experience working independently in a fast-paced environment.
  • Strong technical proficiency in cloud-based systems and clinical platforms.

Experience
  • Experience in ambulatory care or outpatient settings focused on chronic disease management.
  • Inpatient or acute care pharmacy experience is preferred.
  • Background working within health plans or managed care environments.

Schedule / Shift
9:00am-6:30pm Pacific or 6:00am-2:30pm Pacific.

Eligibility for health benefits is based on verifying that an average of 30 hours per week during the first 4 weeks of the work assignment has been met. If you meet eligibility requirements and take action to enroll, you will be covered no earlier than 60 days into your assignment, depending on plan selection(s)).
  • 401k (Eligible on the first 401k open enrollment date following 6 consecutive months on assignment. 401k Open Enrollment dates are 1/1, 4/1, 7/1, and 10/1).
