About the Role

As a Clinical Pharmacist with PharmD Live, you'll engage directly with patients to deliver proactive, pharmacist-led clinical care that improves outcomes and drives value-based success. You'll provide services such as Chronic Care Management (CCM), Transitions of Care Management (TCM),
Remote Patient Monitoring (RPM), and Medication Therapy Management (MTM) through secure telehealth platforms, ensuring each patient receives personalized, evidence-based support.


Key Responsibilities

  • Deliver disease and medication management services via phone or telehealth (video).
  • Conduct comprehensive medication reviews to identify therapy gaps and medication-related issues.
  • Monitor drug therapy for contraindications, drug-drug or food interactions, and dosing accuracy.
