Clinical Liaison

Job Summary:

We are seeking a highly skilled and compassionate Clinical Liaison to join our team at Reliant Care Management Company for the Kansas City, MO area (remote with onsite visits when needed). As a Clinical Liaison, you will play a vital role in facilitating efficient and effective communication between our healthcare facilities and healthcare providers to ensure seamless patient care. Key Responsibilities:

โ€ข Develop and maintain strong relationships with healthcare providers, facilities, and other stakeholders to ensure effective communication and coordination of patient care
โ€ข Conduct clinical interviews with patients to gather information and assess their needs, goals, and preferences
โ€ข Identify and address potential barriers to care, such as social, emotional, or environmental factors
โ€ข Collaborate with healthcare providers to develop and implement individualized care plans, ensuring seamless transitions between care settings
โ€ข Document patient information and communication with healthcare providers in a timely and accurate manner
โ€ข Identify opportunities for quality improvement and participate in quality assessment and improvement initiatives
โ€ข Participate in training and education programs to stay current with industry developments and best practices

Requirements:

  • Must be an LPN or RN

  • 1+ years of experience in a healthcare setting
