Salvo Health is looking for an experienced Lincensed Clinical Social Worker to play an integral role on our care team.
This role will work directly with patients virtually, offering personalized psychosocial support and counseling to patients to reduce symptoms and improve quality of life. The LCSW will empower patients with the knowledge and tools to make lasting behavioral changes that will improve clinical outcomes and overall wellness. You'll serve as one of our patients' primary success partners, with responsibilities including: addressing member health questions, providing relevant educational resources, and fostering positive member accountability. Responsibilities:
  • Provide case consultation and collaborate with interdisciplinary care teams.
  • Conduct case management, including assessing patient needs, coordinating care, and connecting patients with community resources.
  • Lead group sessions focused on psychoeducation, stress management, mindfulness, and meditation.
  • Conduct one-on-one patient visits to provide emotional support, stress management techniques, psychoeducation, and coping strategies.
  • Document patient encounters and coordinating follow-up care to support positive health outcomes.

Requirements:
  • LCSW license with licensure to practice in GA, FL, and MI (also open to PA, NY, OH)
  • 2+ years experience as a an LCSW
  • Bachelor's and Master's degrees required
  • Experience communicating with patients asynchronously preferred
  • Experience with GI conditions or weight management a plus
  • Proficiency in using telehealth platforms and electronic health record (EHR) systems
  • Passion for working with clients to facilitate behavior change by helping them set health goals to effectively manage chronic disease
  • Experience working with patients in a clinic setting preferred
  • Demonstrates a willingness to go above and beyond normal responsibilities to provide the best care possible
  • Excellent interpersonal communication and problem-solving skills
