• Conduct thorough and compassionate assessments of clients' emotional and psychological needs, including historical context regarding substance use and health issues.
• Craft personalized care plans that address both immediate concerns and long-term mental health care, encompassing strategies that support recovery from substance abuse.
• Facilitate various forms of therapy, including individual counseling and group sessions, utilizing evidence-based methodologies such as cognitive-behavioral therapy (CBT) and motivational interviewing (MI) tailored to hospice care.
• Collaborate closely with a team of healthcare professionals including nurses, physicians, and case managers to coordinate an integrated and holistic approach to care.
• Engage families through education and support, helping them understand the emotional dynamics that accompany addiction and terminal illness, and promoting their involvement in care processes.
• Ensure the maintenance of accurate client records in compliance with ethical guidelines while emphasizing confidentiality and respect for client privacy.

As a Licensed Clinical Social Worker at Pro-Health Counseling Services, you will primarily work with clients facing the dual challenges of terminal illness and substance abuse.
