Job description

Salary: $25-$27/hour

The HRSN Housing Case Manager provides comprehensive housing-focused case management services to households experiencing housing instability or crisis. This role supports participants receiving Health-Related Social Needs (HRSN) funding by assisting with housing search, financial assistance coordination, and ongoing stabilization services to promote long-term housing success.


The position requires strong housing navigation skills, landlord engagement, financial documentation oversight, and trauma-informed support practices.


Essential Duties

  • Provides housing navigation and placement support to individuals and families experiencing housing instability.
  • Coordinates financial assistance in compliance with HRSN funding requirements and internal financial protocols.
  • Delivers ongoing case management services to promote housing retention and long-term stability.


Job Functions

  • Assists participants in identifying safe, stable housing options aligned with their long-term goals.
  • Develops individualized, participant-driven housing stabilization plans.
  • Engages landlords, property managers, and housing providers to secure housing placements.
  • Advocates for participants in housing negotiations, lease processes, and tenant rights education.
  • Coordinates financial assistance for rent, move-in costs, utilities, and other eligible housing-related expenses.
  • Ensures proper documentation and compliance with HRSN financial management and billing protocols.
  • Supports participants in accessing rental arrears assistance, utility programs, and other stabilization resources.
  • Provides ongoing case management including goal-setting, life skills coaching, and resource navigation.
  • Coordinates with healthcare providers, social service agencies, and employment programs to provide wraparound support.
  • Assists participants in obtaining identification, enrolling in benefits, and accessing supportive services.
  • Maintains accurate and timely case notes and documentation in HMIS or other required systems.
  • Tracks housing outcomes and participant progress to meet program reporting requirements.
  • Ensures confidentiality and compliance with Medicaid, county, HUD, and agency regulations.
  • Adheres to agency policies and best practices in trauma-informed care, cultural responsiveness, and harm reduction.
  • Performs additional duties as assigned in support of program operations.


Qualifications

  • High school diploma or equivalent required.
  • Strong organizational and communication skills.
  • Ability to maintain confidentiality and manage sensitive participant information.
  • Proficiency in Microsoft Office, Google Suite, and client databases (HMIS preferred).
  • Valid drivers license, proof of insurance, and reliable vehicle required.


Preferred Qualifications

  • Bachelors degree in social work, human services, or related field.
  • Experience working with individuals experiencing homelessness or housing instability.
  • Prior case management experience in housing, supportive services, or Medicaid-funded programs.
  • Bilingual skills (Spanish, Russian, or other languages commonly spoken by participants) preferred.
