Signature Albany Hospice is seeking a skilled and compassionate Registered Nurse (RN) Case Manager to provide high-quality Hospice care to patients in the comfort of their homes. This full-time role is ideal for an RN with experience in Hospice, Home Health, care coordination, and patient-centered care, who thrives in an independent, field-based environment while collaborating with an interdisciplinary team.

What Youโ€™ll DoPatient Care & Case Management
  • Perform comprehensive nursing assessments and develop individualized plans of care
  • Coordinate and manage hospice services in collaboration with physicians and care teams
  • Provide skilled nursing care, treatments, and patient education
  • Monitor patient progress and communicate changes in condition to physicians
  • Help ensure high-quality, compassionate hospice services are delivered consistently
Care Coordination & Documentation
  • Maintain accurate, timely clinical documentation and patient records
  • Communicate with healthcare providers and community resources
  • Ensure access to necessary medical equipment and services
Leadership & Supervision
  • Supervise and evaluate Home Health Aides (HHAs), CNAs, and LPNs/LVNs
  • Provide guidance to ensure quality care delivery and compliance
  • Additional Responsibilities: Participate in IDG, quality assurance and performance improvement programs
  • Attend training and in-service education
  • Participate in on-call rotation as required
What Youโ€™ll Need
  • Graduate of an accredited nursing program
  • Active Registered Nurse (RN) license - Oregon
  • Minimum 2 years nursing experience (hospice, home health or public health preferred)
  • Current CPR certification
  • Valid driverโ€™s license and reliable, insured vehicle
