Summary
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. This position is for the Nursing Home and Adult Day Health Care Case Manager (CNH/ADHC) Registered Nurse (RN) located throughout the Dallas/Ft. Worth metroplex and surrounding areas. The Community Nursing Home Case Manager works in collaboration with Social Worker, to ensure that Veterans in community care facilities are receiving quality care in a safe and patient centered care environment. The CNH/ADHC Case Manager is required to have a state and federal driver's license to operate a government vehicle.
Duties include but are not limited to:
  • Conducts site visits at least every 45 days or more frequently as determined by the CNH Review Team that care and condition sites within the contracted Community Nursing Homes are meeting both the state and the VA quality standards.
  • Acts as liaison between the Dallas And Bonham VAMC and community care facilities on issues of care, veteran satisfaction, contract oversight, family concerns, and provision of service by both the community and VA.
  • Provides and maintain program oversight to ensure quality services and compliance with policy and procedures.
  • Is a member of the Community Nursing Home Oversight Committee.
  • Provides input into monthly visits and annual survey scheduling as well as maintaining accurate census.
  • Assess Veteran and caregiver issues and seeks positive resolution and outcomes.
  • Reports issues where care is not being adequately delivered or Veteran self-determination is jeopardized, to the contract facility and VA administration.
  • Collaborates with others to ensure needed services are performed.
  • Appropriately documents when collaborating with patient's families, all staff, and management at a professional level.
  • Timely communication and documentation of services are required.
  • Knowledge of community resources and ability to serve as a liaison/consultant for nursing staff, and other hospital services regarding continuity of care.
  • Acts as a patient advocate and assist the patient/families to obtain appropriate consultation.

Qualifications
Basic Requirements:
  • English Language Proficiency: In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Education: Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
  • License: Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)

NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification.
Preferred Experience:
  • BSN or above.
  • Three to five (3-5) years Med Surg and Home Care experience.
  • Three (3) years Case Manager Rehab experience.
  • Computer Experience documenting on CPRS, Point Click Care, American Health Tech
  • Exceptional communication and problem solving strengths, collaborating with various disciplines from the Nursing Home and Adult Day Health Care.
  • Familiarity with Medicare/Medicaid regulatory and state guidelines for Community Nursing Homes.
  • Experience in reimbursement of Long Term Care nursing facilities based on Resource Utilization Groups (RUG) and Minimum Data Set (MDS).

Physical Requirements: Must demonstrate medical qualifications to operate a motor vehicle safely: Must possess a current Driver's License.
