This role is designed for a remote setting within a hospice care facility, allowing flexibility while you provide vital services to clients in need. • Conduct thorough assessments that identify social, emotional, and psychological needs of clients within the hospice care context.
• Provide tailored individual and group counseling sessions that utilize evidence-based practices to enhance client coping mechanisms.
• Develop and implement personalized service plans that align with clients' life goals and objectives, addressing both immediate and long-term needs.
• Serve as a strong advocate for clients, ensuring access to necessary resources and upholding their rights in the face of systemic challenges.
• Contribute to program development and enhancement, including evaluating program outcomes and assisting with grant reporting.
• Collaborate effectively with community organizations, government agencies, and volunteers to strengthen the support network for clients.
• Engage in regular team meetings, training sessions, and community outreach initiatives to promote ongoing learning and community education.

The primary focus of your work will be on individuals and families who are experiencing life-limiting illnesses and those facing end-of-life challenges. You will provide compassionate support to help clients navigate grief, loss, and transition, ensuring high-quality emotional care throughout their journey. Understanding the unique dynamics and needs of each client will be critical in delivering effective therapeutic guidance tailored to their circumstances.

• A Master's degree in Social Work from an accredited institution.
• Active and unrestricted state licensure as a Licensed Clinical Social Worker (LCSW).
• Proficiency in relevant therapeutic techniques and methodologies suited for terminally ill populations.
• Demonstrated commitment to social justice, advocacy, and trauma-informed care practices.
• Exceptional communication, empathy, and organizational skills that foster effective client relationships and outcomes.

• A minimum of 2 years’ experience working within a nonprofit organization or similar social services environment preferred.
• Experience in hospice or palliative care settings is highly desirable.
• Knowledge of local community resources to assist clients in accessing support services.
