Key Responsibilities
  • Conduct comprehensive evaluations and ongoing assessments to ensure that each patient receives personalized care tailored to their unique needs.
  • Facilitate effective communication among supervisors, care providers, family members, and relevant stakeholders to track patient progress and enhance the treatment plan.
  • Prepare and share a structured outline with supervisors, detailing appointments, home visits, and sessions to monitor and assess the efficacy of the intervention strategies being employed.
  • Organize and execute home visits that involve in-depth observations, chart reviews, and consultations with families, ensuring a holistic understanding of the client’s environment.
  • Ensure the adherence to and effectiveness of program strategies, regularly participating in interdisciplinary team meetings to collaborate on best practices and share insights.
  • Document monthly and quarterly progress notes meticulously, keeping accurate records of patient interactions and treatment developments.
  • Develop and conduct training sessions to enhance the skills of team members and ensure compliance with paperwork and program objectives.
  • Engage in periodic follow-up interviews and home visits to reaffirm that ongoing patient needs are being met efficiently.
  • Actively assist in outreach to the community to strengthen the connection between mental health resources and the public.
