  • Maintain accurate case documentation, ensuring all reports and data submissions are completed in a timely manner, meeting organizational and regulatory standards.
  • Assist with coordinating care transitions and continuous monitoring of client status to ensure adherence to treatment goals.
  • Contribute to strategic committees aimed at enhancing service delivery and promoting quality improvement across the organization.

This includes collaborating with patients from diverse backgrounds, respecting their cultural beliefs and practices. Understanding the emotional complexities associated with end-of-life care is essential to providing appropriate support and guidance.

  • Strong organizational, planning, and interpersonal skills promoting collaboration within diverse teams.
  • Proficient in communication, both verbal and written, enabling effective interaction with patients, families, and providers.
Preferred Experience
  • At least two years of clinical experience, particularly in a hospice or healthcare setting.
  • Background in trauma-informed care or similar therapeutic approaches.
Compensation and Benefits

This position offers a competitive salary ranging from USD 70,000 to 85,000 per year, commensurate with experience.
