• Develop and implement personalized support plans targeting the unique needs of hospice clients, addressing their psychological and emotional challenges related to illness.
• Conduct thorough assessments through interviews and use evidence-based practice to gauge clients’ current conditions, needs, and available strengths.
• Work collaboratively with clients to set individualized, measurable goals, ensuring clients have a clear understanding of their treatment objectives and expected outcomes.
• Monitor client progress and adapt treatment plans in response to ongoing assessments and feedback during therapy sessions.
• Provide expert guidance on coping strategies and resources during counseling sessions, fostering empowerment among clients and their families.
• Maintain meticulous records of client consultations and progress reports, which are crucial for compliance and case continuity.
• Attend and contribute to case conferences, providing insights and updates on client status to ensure integrated care across all service providers.
• Act as a key worker, connecting clients with relevant community resources that support their journey towards recovery and improved quality of life.

Your work will primarily focus on individuals diagnosed with life-limiting conditions and their families, addressing their emotional and psychological needs during a challenging time. As a Licensed Clinical Social Worker at Archway Counseling Center, you will navigate sensitive conversations tailored to the unique circumstances of each client, guiding them through critical decisions and transitions with care.

• A valid social work license (LCSW) issued by the state of Texas, with certification in clinical social work preferred.
• A Bachelor’s Degree in Social Work (BSW), Psychology, or Sociology. A Master’s Degree in Social Work (MSW) is strongly preferred.
• Proven experience as a Social Worker, particularly in clinical or therapeutic settings.
• Deep understanding of social work principles and practices, with a strong foundation in social theories.
• Exceptional interpersonal skills, capable of building and maintaining trusting and professional relationships with clients and colleagues.
• Ability to communicate effectively and sensitively with a diverse client population, adapting your approach according to individual needs.
• Strong problem-solving skills and resilience in assessing and responding to complex situations.

• Experience within a hospice or palliative care setting is highly desirable.
• Familiarity with telehealth systems and remote counseling methodologies.
