Key Responsibilities

• Conduct comprehensive clinical assessments for patients entering hospice care to better understand their mental and emotional needs.
• Devise individualized treatment plans that incorporate evidence-based therapeutic approaches tailored to unique patient circumstances.
• Implement solution-focused interventions and therapeutic techniques that address emotional, psychological, and social issues related to terminal illness.
• Maintain meticulous, up-to-date clinical documentation in compliance with HIPAA and other regulatory requirements.
• Engage with a collaborative team of healthcare professionals including nurses, physicians, and chaplains, to coordinate holistic care.
• Participate in case consultations and peer review meetings to elevate the overall quality of care provided.
• Empower patients and their families by educating them about mental health resources and supportive strategies throughout the hospice experience.
• Stay informed of the latest research and trends in social work and hospice care to enhance intervention efficacy.

Patient Population / Client Focus

In this role, you will primarily work with diverse patients facing terminal illnesses, along with their families. Your focus will involve addressing complex emotional and psychological challenges during the end-of-life journey, enhancing patients' overall well-being and offering vital support to their loved ones.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• A valid LCSW license to practice in the state of Tennessee.
• A minimum of three years of direct clinical experience in psychotherapy, particularly with a focus on mental health in palliative or hospice settings.
• Strong proficiency in solution-focused therapy interventions and therapeutic modalities.
• Exceptional communication, interpersonal skills, and emotional intelligence necessary for working in high-stress environments.
