• Conduct thorough assessments to gauge mental health needs and consider the impact of any medical conditions on patients' mental wellness.
• Formulate and implement integrated treatment plans that effectively address both psychiatric and medical issues, tailoring approaches to individual patient circumstances.
• Lead individual and group therapy sessions that are evidence-based and focused on personal growth, resilience, and achieving therapeutic goals.
• Work in tandem with medical providers, ensuring coordinated care that enhances treatment outcomes and addresses all facets of patients’ health.
• Provide crisis intervention services for patients experiencing mental health emergencies, ensuring immediate and effective support.
• Educate patients about the vital connection between mental health and physical health, equipping them with tools for self-management of their conditions.
• Maintain meticulous and confidential patient records in compliance with legal and ethical standards, leveraging electronic health record systems effectively.
• Participate in continuous professional development by staying informed about the latest research and practices in integrated mental health care.

As a Licensed Mental Health Therapist with us, you will mainly work with individuals dealing with co-occurring medical conditions such as autoimmune disorders, chronic pain, and substance use disorders. This diverse patient demographic requires tailored approaches to therapy, and you will support them in managing not just their mental health, but also the interplay between their physical and psychological well-being.

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Active licensure as an LCSW, LCSW-R, LMFT, or LMHC in the state of New York.
• Demonstrated experience in addressing the mental health needs of patients with co-occurring medical conditions.
• Exceptional interpersonal and communication skills necessary for effective patient interactions.
• Strong adherence to professional ethics and confidentiality protocols.
• Proficiency in using computer applications and electronic health record systems to manage patient information.
• Capability to collaborate seamlessly within an interdisciplinary team.

• Experience in outpatient settings with a focus on integrated care practices.
• Familiarity with evidence-based therapeutic modalities, particularly in group therapy settings.
• Previous experience providing crisis intervention and support.
