Key Responsibilities
  • Conduct comprehensive assessments to identify the unique mental health needs of each patient.
  • Facilitate effective communication between supervisors, and direct care staff, as well as families and guardians to monitor patient progress and ensure continuity of care.
  • Develop and manage a tentative schedule for appointments and home visits to ensure consistent patient engagement.
  • Coordinate and implement home visits, which include observational assessments, chart reviews, and comprehensive data analysis, to support patients in their natural environments.
  • Oversee the execution of individualized program plans, ensuring workflows are effective and align with best practices in treatment.
  • Engage in regular interdisciplinary team meetings to collaborate on treatment strategies and patient progress updates.
  • Prepare and submit monthly and quarterly progress notes, maintaining accurate and up-to-date documentation as required by healthcare guidelines.
  • Organize training sessions to ensure that team members understand program implementation and documentation protocols.
  • Provide ongoing follow-up and support through interviews and home visits to promote sustained patient progress.
  • Maintain thorough records to ensure relevant information is accessible for stakeholders involved in patient care.
