• Conduct comprehensive psychosocial assessments for clients and their families upon admission and as clinically indicated, utilizing relevant social work theories to identify and address needs.
• Collaborate effectively with interdisciplinary healthcare teams to formulate, execute, and revise personalized psychosocial treatment plans, ensuring consistent communication and continuity of care.
• Facilitate supportive and crisis intervention counseling for patients and families, guiding them through emotional challenges related to illness.
• Manage a diverse caseload with varying degrees of clinical acuity, utilizing sound clinical judgment to prioritize and address individuals' needs.
• Conduct case finding initiatives for patients with potential psychosocial concerns by actively participating in team conferences and interdisciplinary rounds.
• Deliver psychoeducation to clients and families about adapting to diagnoses, treatment options, and navigating transitions, such as the shift from youth to adult services.
• Contribute to quality assurance initiatives and community improvement efforts concerning mental health services by taking part in the review and development of social work policies and procedures.
• Provide supervision and mentorship to MSW students and interns, fostering a learning environment to enhance their clinical skills.
• Maintain accurate and timely electronic medical records in line with organizational policies, which includes generating written reports and transitioning case hand-offs efficiently.

As a Licensed Mental Health Counselor with Harbor of Hope Counseling Services, you will be serving a diverse range of clients, including adults, adolescents, pediatric populations, and geriatric patients. Our goal is to address the unique psychosocial challenges faced by these groups, ensuring they receive holistic care that respects their individual experiences and histories.

• A valid license to practice in the state of California, such as LCSW, LMFT, or LPCC.
• A master's degree in a behavioral science or related field from an accredited institution is required.
• Strong written and verbal communication skills, particularly in case documentation and report writing.
• In-depth knowledge of effective assessment and counseling techniques, and a solid foundation in social work principles.

• Previous experience working in outpatient settings, focusing on diverse patient populations.
• Familiarity with electronic health record systems and documentation protocols in mental health settings.
• Experience providing supervision or mentorship to developing clinicians and students.

• Annual salary range of USD 120,000 – 130,000, commensurate with experience and qualifications.
