Job Summary:

Isle Health and Rehabilitation is seeking a dedicated and skilled MDS Remote to join our team of healthcare professionals. As a vital member of our clinical team, you will play a critical role in ensuring the highest level of care and quality outcomes for our residents. Responsibilities:

  • Conduct MDS assessments, RAI assessments, and Care Plans to ensure accurate and comprehensive documentation

  • Coordinate with interdisciplinary teams to develop and implement individualized care plans

  • Review and update resident records, ensuring compliance with regulatory requirements and company policies

  • Provide education and training to staff on MDS and RAI protocols

  • Participate in quarterly MDS QAPI meetings to review and improve MDS processes

  • Collaborate with other departments to ensure seamless communication and coordination

  • Perform other duties as assigned to ensure the delivery of high-quality resident care
