Work Environment

As a remote position within our Hospice Center, you will engage with patients primarily through telehealth capabilities, allowing you flexibility while ensuring constant connection with the rehabilitation team. Key Responsibilities

• Conduct comprehensive psychosocial assessments utilizing interviews, observations, and the review of medical records to determine the emotional, social, and environmental factors affecting patient recovery.
• Provide empathetic counseling and support to patients and their families, aiding them through emotional transitions and mental health challenges during rehabilitation.
• Collaborate with a diverse rehabilitation team to develop individualized treatment plans tailored to the psychosocial needs and goals of each patient.
• Facilitate access to community resources, including support groups, vocational rehabilitation services, and housing assistance, to ensure holistic support for patients.
• Educate patients and their families on the available community resources and post-rehabilitation services to ensure a smooth transition after discharge.
• Participate actively in discharge planning processes, ensuring a seamless transition from our Hospice Center to the patient’s home or other care facilities.

Patient Population / Client Focus

Your primary focus will be on patients at our Hospice Center, many of whom are facing significant life challenges stemming from severe illness or disability. Qualifications

• Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• In-depth understanding of psychosocial dynamics affecting patients in rehabilitation settings.
• Excellent communication and interpersonal skills with the ability to connect with diverse patient populations.

Preferred Experience

• Previous experience working in rehabilitation or medical settings, ideally within hospice, palliative care, or long-term care environments.
• Familiarity with community resources and support networks pertinent to disabled and chronically ill populations.
• Strong background in grief counseling and coping strategies.

Compensation and Benefits
