We are seeking a dedicated and compassionate Licensed Professional Counselor (LPC) to join the Mohawk Trail Counseling Center in Redmond, Oregon. In this remote outpatient role, you will collaborate with a multidisciplinary team of healthcare professionals. Despite the virtual setting, you will maintain a close working relationship with primary care providers and other specialists, contributing to integrated patient care. The flexibility of remote work allows for better work-life balance while maintaining a structured schedule based on patient needs and appointment timings.

• Conduct comprehensive patient intakes, focusing on diagnostic evaluations and functional assessments to guide treatment planning.
• Provide brief, focused interventions, offering immediate support to patients requiring mental health services.
• Collaborate with primary care providers to identify patients in need of specialized mental health care and facilitate appropriate referrals.
• Initiate follow-up care to monitor patient progression and make necessary adjustments to treatment plans.
• Assist in developing personalized relapse prevention plans for patients, ensuring they can maintain stable functioning.
• Detect high-risk patients and devise plans to mitigate the escalation of mental health issues.
• Document patient progress in treatment charts meticulously and communicate effectively with other healthcare providers regarding patient needs.
• Support patients in adhering to medical treatments initiated by primary care providers, including strategies for managing medication side effects.
• Provide educational resources that empower patients to participate actively in their treatment and recovery journeys.

Your role will primarily focus on adults experiencing a range of behavioral health concerns, including anxiety, depression, trauma, and substance use challenges. Understanding the socio-economic and cultural backgrounds of your patients is essential, as your interventions will be tailored to accommodate diverse needs. You will play an integral role in promoting mental wellness and ensuring that clients receive holistic care within a supportive environment.

• A Master's Degree in Clinical Counseling or a related field is essential.
• Current LPC (Licensed Professional Counselor) license.
• Thorough knowledge of mental health assessment methods, treatment planning, and therapeutic techniques.
• Proficiency in psychodiagnostics and a comprehensive understanding of developmental psychopathology.
• A strong ability to establish trustful relationships with clients and collaborate effectively with an interdisciplinary team.
• Solid organizational skills for maintaining accurate clinical records and preparing insightful reports.
• Ability to exercise professional judgment and decision-making based on evidence-based practices.

• Bi-lingual proficiency in English and Spanish is preferred but not mandatory.
• Previous experience working in a primary care setting or community health organization is advantageous.
• Familiarity with community resources and their utilization to address behavioral health issues in patients.
