• Facilitate crisis intervention sessions, offering immediate emotional support to patients and families in high-stress situations.
• Coordinate post-discharge services by connecting patients with community resources to ensure continuity of care beyond the hospital environment.
• Participate actively in multidisciplinary team meetings, sharing insights into psychosocial factors that affect patient outcomes.
• Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with hospital protocols and state regulations.
• Educate patients and families on available treatment options, resources, and coping mechanisms to empower them through their healthcare journey.
• Contribute to hospital-wide initiatives focused on continuous improvement and quality of care delivery.
• Support research initiatives related to psychosocial care and participate in educational opportunities to enhance professional skills.

You will primarily work with a diverse patient population, including individuals facing chronic and acute health issues, mental health disorders, and social vulnerabilities. This role places a strong emphasis on Understanding the psychosocial influences on healthcare adherence and outcomes

• A Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) within Florida.
• Strong foundational knowledge of medical terminology and an understanding of mental health diagnostics.
• Exceptional communication and interpersonal skills, able to engage diverse populations with empathy and professionalism.
• Ability to maintain confidentiality and handle sensitive information in accordance with HIPAA regulations.

• Two years of clinical experience in a healthcare setting, particularly in hospitals or community health environments.
• Experience with electronic health record (EHR) systems and documentation standards.
• Familiarity with evidence-based practices in crisis intervention and social work methodologies.
