About the Job:
Title: Registered Nurse Ambulatory - Anticoagulation Clinic
As a Registered Nurse in the Anticoagulation Clinic, you will provide specialized outpatient care to patients receiving anticoagulation therapy. You'll collaborate with providers to monitor and manage anticoagulant medications (e.g., warfarin, DOACs), perform point-of-care INR testing, assess bleeding and clotting risks, and educate patients on medication safety and lifestyle considerations. This role requires strong clinical judgment, patient communication, and a proactive approach to chronic disease management.
What You'll Do:
  • Provide nursing care and education to patients on anticoagulant therapy.
  • Monitor and adjust anticoagulation plans under provider guidance.
  • Perform INR testing and interpret results to ensure safe therapeutic levels.
  • Educate patients on medication adherence, dietary interactions, and symptom monitoring.
  • Communicate with providers regarding dosage adjustments and care plans.
  • Document assessments, interventions, and patient interactions in the EMR.
  • Support quality improvement initiatives and patient safety protocols.
  • Work closely with interdisciplinary teams to ensure continuity of care.

Who You Are:
  • Graduate of an accredited nursing program (ASN required, BSN preferred).
  • Licensed RN in NH, MA, or compact state, with current BLS/CPR certification.
  • Prior experience in ambulatory care, anticoagulation management, or chronic care preferred.
  • Strong communication and clinical assessment skills.
  • Comfortable working independently and collaboratively in a fast-paced setting.
  • Knowledge of EMR systems (EPIC preferred) and patient-centered care practices.
