AuthoraCare Collective is currently seeking an RN for our After-hours Triage department supporting our Burlington and Greensboro service areas.

This is a remote part-time position; Every Monday and Tuesday 4:45pm - 9:45pm. Must reside in the Burlington/Greensboro, NC area.

CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Job Summary:

Triage calls received from patients/family members/facilities/providers to assess needs. Assign visits to team members as needed. Assess and identify patient/caregiver problems, establish patient/caregiver care goals, and identify and implement interventions for all agency programs.

Work with providers to establish and implement plan of care for the patient. Work closely with members of the interdisciplinary team and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations.
