Job description

DUTIES AND RESPONSIBILITIES OF THIS POSITION:
Direct Support Professionals will provide in-home residential service to individuals with intellectual or developmental disabilities. Our team needs someone who is fun energetic and enjoy working one on one with our individuals in their own homes to allow individuals to accomplish their goals in the least restrictive setting.
  • Support individuals with budgeting and managing their money.
  • Guide individuals with completing necessary hygienic and household tasks.
  • Engage individuals in social activities.
  • Support individuals with community access.
  • Ensure individual health and safety is maintained.
  • Complete daily progress note describing the progress of the individual each day.
  • Complete quarterly reports describing individual progress.
  • Follow outcomes and goals on Individualized Service Plans (ISPs).
  • Attend in-service training sessions and workshops for professional development.
  • Attend biweekly supervision sessions documenting individual progress.
  • Maintain client confidentiality in all settings.
  • Facilitate conflict resolution as needed.
  • Attend physician appointments, SIS assessments, and Annuals related to individual goals.

QUALIFICATIONS: Direct Support Professionals will be expected to present and maintain specific qualifications for our agency. These qualifications include but are not limited to:
  • High school diploma at minimum or equivalent course work
  • One year experience caring for individuals with developmental or physical disabilities
  • A valid driver's license
  • Current and valid auto insurance
  • Reliable computer for documentation completion
  • Current negative two part PPD test
  • CPR/First Aid
  • Medicaid Waiver Competency
  • Eligible Criminal Background Check
  • Eligible Central Registry Check

MINIMUM SKILLS, KNOWLEDGE AND ABILITIES: Direct Support Professionals will be expected to demonstrate necessary skills and knowledge in regards to supporting our individuals. At minimum they must:
  • Understanding of the medical field and knowledge of terminology which includes working directly with families, physicians, therapists, and pharmacists.
  • Demonstrates organizational and problem solving skills.
  • Ability to be flexible.
  • We will provide as applicable: TOVA, First Aid and CPR, Medication Management, Human Rights/ID Waiver, Emergency Preparedness, Person Centered Training ad Serious Incident Training.
  • Knowledge of regulatory agency for this services - DMAS and DBHDS.
