Job Summary

We are seeking an experienced Director of Case Management to lead and oversee hospital case management operations. This leadership role is responsible for ensuring effective care coordination, discharge planning, regulatory compliance, and optimal utilization of clinical resources. The ideal candidate will bring strong leadership, strategic thinking, and the ability to drive performance outcomes in a fast-paced acute care environment.
