safe mobility techniques

• Participate in care planning meetings, utilization reviews, and MDS documentation as

applicable

• Maintain timely, accurate clinical documentation in compliance with Medicare/Medicaid and

facility requirements

• Uphold resident rights, dignity, and confidentiality in all interactions

• Assist in maintaining a clean, organized, and safe therapy gym and treatment environment

QUALIFICATIONS

Required:

• Associate's degree from an accredited PTA program

• Current, unrestricted PTA licensure in the State of Colorado (or ability to obtain prior to start

date)

• Current CPR/BLS certification

• Strong interpersonal and communication skills with a patient-first mindset

Preferred:

• 1+ years of experience in a skilled nursing, long-term care, or post-acute rehabilitation

setting

• Familiarity with MDS/PDPM documentation frameworks

• Experience with geriatric populations and fall prevention programs

• Knowledge of EMR/documentation systems used in SNF environments

WHY JOIN OUR TEAM?

• Competitive compensation package commensurate with experience

• Housing assistance and relocation support for candidates relocating to the Ordway area

• Health, dental, and vision benefits

• Paid time off and holiday pay

• Supportive, collaborative care team with low therapist-to-patient ratios
