Job description

The Case Manager is responsible for:
  • Assessing the areas of the consumerโ€™s life where assistance is needed.
  • Offering and coordinating resources, and services for the person.
  • Monitoring service delivery and working with providers to ensure the consumerโ€™s needs are met.
  • Provide person centered service planning with person and creating Coordinated Consumer Support Plans.
  • Attend regular meetings with the person and their interdisciplinary teams.
  • Travel to different sites in different counties for meetings.
  • Case managers will be providing case management for people on EW, AC, RULE 185, CADI, CAC, and IDD waivers.
  • Other duties as assigned.
Case Manager Requirements:
ยท Applicants must have a valid driverโ€™s license and acceptable driving record as determined by Bestview Care Options policy. Applicants must have a vehicle that is covered with current insurance. Applicant must have a reliable vehicle.
ยท Applicants must successfully clear local and/or stated background checks.
ยท โ€˜Preferred Qualifications: One to two yearsโ€™ experience in service coordination or case management is preferred, but not required.
ยท Applicants will be required to have internet at home.
