St. Cloud Pediatrics (SCP) Office | Minnesota

Employment type: Full time
Salary: $80,000–$90,000 per year, paid weekly

Work arrangement: Hybrid home-and-field role; significant field presence required

This is a hybrid home-and-field position with significant travel throughout St. Louis County to support clients, clinicians, and care delivery. This is not a fully remote position.

Do you have experience supervising, mentoring, or developing clinicians in a home care, pediatric, or skilled nursing setting? Are you looking for a flexible schedule that combines hands-on field leadership with focused work-from-home time?

BAYADA Home Health Care is seeking an RN Pediatric Clinical Manager for the St. Cloud Pediatrics (SCP) office in Minnesota. The office provides adult and pediatric skilled nursing services as well as pediatric personal care services.

In this role, you will help ensure that medically fragile clients receive safe, consistent, and compassionate care in their homes. You will serve as a clinical resource for field employees, a trusted partner to clients and families, and a key member of the office leadership team. Your leadership will help strengthen continuity of care, support clinician success, and uphold BAYADA’s professional, ethical, and safety standards.

Our Skilled Nursing practice provides expert, compassionate care to medically fragile children and adults in the home. Care may include complex needs such as tracheostomy and ventilator support, seizure management, and tube feedings.

Position summary

The Clinical Manager leads field staff in delivering excellent client service, clinical care, quality, and adherence to policies and procedures. The role combines clinical leadership, case management, staff development, client and family support, and operational partnership.

The successful candidate will be comfortable moving between the field and home-based work. Some days may focus on client visits, assessments, supervisory visits, clinician coaching, or care coordination. Other days may include documentation, case review, care-plan updates, staffing collaboration, and follow-up with clients, families, clinicians, and office leadership.

What the hybrid role looks like

Hybrid flexibility is built into the position to support both responsive clinical leadership and efficient administrative work. The role requires regular in-person engagement and travel throughout the service area.

In the field

  • Visit clients in their homes to conduct assessments, evaluate care needs, and support the care planning process.
  • Complete supervisory and quality visits with field clinicians.
  • Observe care delivery and provide real-time coaching, education, and clinical guidance.
  • Build trusting relationships with clients, families, caregivers, and clinicians.
  • Support continuity of care for pediatric and adult skilled clients.
  • Respond to changing client needs and help address clinical or service concerns.
  • Travel throughout St. Louis County as needed to support safe, high-quality care delivery.

From home

  • Complete clinical documentation, care plans, assessments, and follow-up activities.
  • Review field employee documentation and clinical performance.
  • Coordinate cases, staffing needs, client services, and interdisciplinary communication.
  • Prepare for client visits and follow up on clinical recommendations.
  • Coach and mentor clinicians through phone, video, and other communication channels.
  • Partner with office leadership on quality, compliance, retention, and service delivery priorities.

Qualifications

  • Current Registered Nurse license in good standing in Minnesota required.
  • Minimum of two years of nursing experience preferred; home health experience may vary based on office need.
  • Experience supervising, mentoring, or developing clinicians in a home care, pediatric, or skilled nursing setting strongly preferred.
  • Trach, ventilator, and tube-feeding experience in a home health care setting preferred.
  • Demonstrated leadership, organization, multitasking, communication, and problem-solving skills.
  • Ability to work independently in a field-based environment while remaining connected to the office and care team.
  • Willingness and ability to travel regularly throughout the assigned service area.
  • Commitment to professional, ethical, and safe clinical practice.
  • Ability to relocate to the service area is required if far away.

  • Full-time position with a flexible schedule.
