Description
The Registered Nurse (RN) provides clinical oversight and nursing services to residents within the company's Community-Based Residential Facilities (CBRF) in accordance with Wisconsin DHS regulations. The RN works collaboratively with leadership and care teams to ensure quality resident care, regulatory compliance, and staff support.
ESSENTIAL JOB RESPONSIBILITIES:
Clinical Oversight & Resident Care
Perform comprehensive nursing assessments and ongoing evaluations of residents.
Develop, review, and update resident care plans, service plans, and health-related documentation.
Coordinate and oversee medication management, including medication reviews and physician communication.
Monitor residents' health status and respond to changes in condition appropriately.
Provide nursing interventions within RN scope of practice and regulations.
Regulatory Compliance
Ensure compliance with Wisconsin DHS regulations and Platinum Communities policies.
Assist with audits, surveys, inspections, and plan of correction development as needed.
Ensure proper documentation and timely completion of required assessments and reports.
Staff Support & Education
Provide clinical guidance, training, and support to administrators, med techs, and caregivers.
Assist with competency evaluations, medication administration training and delegation, and ongoing education.
Serve as a clinical resource for staff regarding resident care and best practices.
Support leadership with clinical initiatives, quality improvement efforts, and special projects.
Care Coordination
Collaborate with physicians, pharmacies, hospices, home health agencies, and other healthcare providers.
Participate in care conferences as required.
Assist with admissions, discharges, and transitions of care as required.
Requirements
• Current, unrestricted Registered Nurse (RN) license in the State of Wisconsin.
• Knowledge of Wisconsin CBRF and RCAC regulations and DHS requirements (or willingness to learn).
• Strong clinical assessment, documentation, and communication skills.
• Valid driver's license and reliable transportation; willingness to travel between communities.
• Occasional on-call or flexible scheduling may be required based on community needs.
Preferred:
• Experience in assisted living, long-term care, or geriatric nursing.
• Prior experience providing clinical oversight across multiple facilities.
• Familiarity with electronic health records and care management systems.
