REENTRY SPECIALIST POSITION SUMMARY:


The Reentry Specialist provides access to community programs, ensures continuity of care following release into the community, advocates for patient needs, and act as liaisons between the multiple systems impacting the lives of offenders with mental health illnesses, co-occurring substance use disorders, and chronic medical illnesses.

REENTRY SPECIALIST MINIMUM REQUIREMENTS:

  • One year of experience in social services field.
  • Bachelor’s degree in Social Work, Counseling, Psychology, or related field
  • Experience working with individuals with Substance Use Disorders
  • Case Management experience
  • Applicants with experience in a correctional health care setting will be given preference.
  • Certification or credential in substance use counseling (e.g., CADC, CSAC, or equivalent) strongly preferred.
  • Knowledge of SUD treatment and recovery principles.
  • Strong communication, organizational, and advocacy skills.
  • Ability to work effectively within a multidisciplinary team.
