Job Summary


Provides direct support for individuals who are in, or seeking recovery services, to include family members, significant others, or any other supportive allies. Recovery Coach will also make necessary referrals, coordinate services, work with the participants to develop skills that improve functioning and make frequent contact with the participant in their home and out in the community to assist with daily living and practical everyday problems. Recovery Coach will promote assertive advocacy, provide transportation, help participants build recovery capital and engage in an overall recovery approach to assist participants to maintain sobriety and achieve long term recovery.


Essential Duties and Responsibilities


• Complete a strength-based assessment of participants' strengths, challenges, and goals in multiple life domains and assist in the development of a recovery plan based on the wants, needs, and interests of the participant.
• Conduct enrollment of participants into the program and maintain accurate, neat, and timely documentation on all case files. Collect necessary data in a timely manner and as determined by the contract.
• Conduct outreach in the community, at schools, and locations where individuals seeking recovery congregate.
• Provide transportation for participants to and from the program site to activities, to referrals, and as needed by the participant.
• Employ brief interventions in the context of a peer-to-peer relationship to engage participants and encourage retention in program services; this may include crisis intervention, relapse prevention, and individual or family consultations.
• Serve as the point of contact through the entire continuum of care from enrollment through treatment and aftercare, including providing information on available community resources, assistance in accessing services, coordination of services, and transportation.
• Introduce and link participants to recovery community organizations, peer-run services, and other community organizations that will assist them in meeting recovery needs and building recovery capital.
• Maintain frequent contact, weekly or as determine by the dosage chart frequency, with participants/families through home visits, internet communication, telephone, or office visits to identify continuous needs associated with the participants' recovery, to monitor progression and/or regression, intervene on a timely basis, and encourage recovery activities.
• Facilitate peer to peer group sessions, as assigned, and in accordance with participant and programmatic needs.
• Establish and facilitate a Peer Leadership Advisory Council in which participants in recovery, their family members, and supportive allies are involved with program design, evaluation, and have opportunities to advise and/or make organizational decisions related to recovery orientation.

• Be knowledgeable and competent in applying the principles of recovery coaching in the context of the program, and those targeted by this program.
• Ability to share personal recovery experiences appropriately and teach participants how to build recovery capital.
• Ability to maintain confidentiality and professional boundaries.
• Ability to effectively interact and work as a team member representing Aliviane Inc.'s core values.
• Ability to work in a collaborative manner with professionals, participants, and the community.
• Must have a clean driving record and be able to drive the program vehicle to transport participants.
