Dedicated to delivering top-tier, accessible, and inclusive primary healthcare, PHS has proudly served for 55 years and remains the largest community health center in central NC. Operating 11 Community Health Centers, two PACE (Program of All-Inclusive Care for the Elderly) SeniorCare facilities, and 2 Mobile Health Units, PHS extends its services to residents across five counties, including Alamance, Caswell, Chatham, Orange, and Lee.

About Position:ย  ***This is a Float position that requires travel between Alamance and Caswell county clinics as needed***

  • Work Schedule:
    • Monday,ย  8:00am - 5:00pm
    • Tuesday,ย  8:00am - 5:00pm
    • Wednesday,ย  8:00am - 8:00pm
    • Thursday,ย  8:00am - 5:00pm
    • Friday,ย  8:00am - 1:00pm

Job Summary: The primary role of the Community Health Worker (CHW) is to be a link/liaison between public health/health related services to the community. The CHW is tied to the community and is able to provide cultural humility and competence to individuals within the community. Help to build capacity, access, and care opportunities through health education, health promotion, direct outreach, community events, social support and advocacy. Work both within the community and within the clinic setting where specific duties may include: assessment and addressing of social determinants of health, providing care coordination, chronic disease management, perinatal care coordination, financial assistance, wellness screenings, etc. Target at risk patients identified via screenings, care gaps, and referrals.

Qualifications:

  • Bachelorโ€™s degree in social work, Human Services, Public Health or related filed or 1year MINIMUM health care experience preferred.
  • Analytical thinking and problem-solving ability
  • Excellent interpersonal, oral and written communication skills
  • Must have strong emotional intelligence
  • Only act within the scope of practice
  • Meet a standardized set of competencies for the position description established by PHS before working independently
  • Current/valid driverโ€™s license, proof of insurance, and reliable vehicle required
  • Preferred Community Health Worker Certification
  • Bilingual English/Spanish preferred

Immunizations:ย Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment

Pay Range: $22.49/Hourly - $30.23/Hourly (commensurate with years of experience)
