Responsibilities:

As a Perinatal Community Health Worker supporting the Espanola NM area you will make a meaningful impact on the health of pregnant and postpartum people and young children as a Perinatal Community Health Worker. This role is embedded in Presbyterian Espanola Hospital, connecting patients to community resources, providing basic health education, supporting care navigation, and leading the Espanola Produce Rx program for perinatal and pediatric participants.

Qualified candidates bring experience in community health or clinical settings, a commitment to community wellbeing, and comfort using Microsoft Office and learning an electronic medical record.

This position works primarily in person at the clinic, with some outreach in Espanola and surrounding communities. It is part of a collaborative statewide Presbyterian Community Health team, with remote meetings and occasional travel for trainings.

This role includes a $4/hour differential to support costofliving needs in the Espanola area.

Key responsibilities include:

  • Following up on clinical and community referrals for pregnant and postpartum people

  • Navigating patients to appointments, resources, and Medicaid enrollment

  • Leading the Espanola Produce Rx program, enrolling eligible participants and coordinating weekly food distribution

  • Building community partnerships and attending events to identify individuals needing support accessing care

  • Documenting referral outcomes in Excel and the EPIC electronic medical record

  • Providing feedback on workflows and standard operating procedures

Qualifications:

Level I qualifications:

  • High school plus specialized, short-term training in clinical, medical skills.

  • Must be able to speak language of the population served.

  • Education Essential: High School Diploma or GED


Level II Qualifications:

  • (Level II and III qualifications) Certified Community Health Worker Specialist I, II, or III; Three years of experience interacting with community members or in community health/public health services.
