Job Description:

Patient Navigation & Care Coordination
Act as the primary point of contact for orthopedic patients from referral to recovery.
Coordinate appointments, diagnostics, pre-operative clearances, and post-operative follow-ups.
Ensure timely movement of patients through the care continuum, minimizing delays.
Educate patients and families on procedures, expectations, recovery plans, and care pathways.
Identify and address barriers to care (transportation, financial, social determinants of health).

Clinical Pathway Management
Support implementation and adherence to standardized orthopedic pathways (e.g., total joint replacement programs).
Monitor patient progress across the continuum and ensure compliance with protocols.
Facilitate prehabilitation and post-acute care coordination (home health, rehab facilities).

Physician & Team Collaboration
Partner with orthopedic surgeons, advanced practice providers, and multidisciplinary teams.
Coordinate interdisciplinary communication to ensure seamless care transitions.
Support physician practices with scheduling optimization and case progression.
Patient Experience & Engagement
Serve as an advocate for patients and families.
Promote patient engagement tools (education materials, digital platforms, classes such as joint camp).

Education
Minimum Requirements --- Graduate of accredited nursing program

Certification/Licensure (including annual requirements)
Minimum Requirements --- Current NC or Multistate RN license; BSN, BLS, ACLS
Preferred at Hire --- ONC, must obtain within 1 year of hire
