Care Coordinator (Full-Time Office Position)Join Our Team!

A&C Private HomeCare is seeking a compassionate, organized, and motivated Care Coordinator to join our growing team. This is an in-office position responsible for coordinating caregiver schedules, supporting clients and caregivers, and ensuring exceptional customer service while maintaining compliance with agency policies.

The ideal candidate is a strong communicator who thrives in a fast-paced environment, is detail-oriented, and enjoys making a positive impact in the lives of others.

Responsibilities
  • Coordinate and maintain caregiver schedules.

  • Match caregivers with clients based on client needs, availability, and caregiver qualifications.

  • Communicate with clients, caregivers, family members, hospitals, and case managers.

  • Complete caregiver onboarding and orientation.

  • Assist with recruitment, interviewing, and hiring of caregivers.

  • Monitor caregiver compliance, credentials, and required documentation.

  • Conduct routine follow-up calls with clients and caregivers to ensure satisfaction.

  • Maintain accurate documentation and electronic client records.

  • Assist with call-outs, schedule changes, and emergency staffing needs.

  • Ensure agency policies, procedures, and state regulations are followed.

  • Work closely with the management team to promote quality care and excellent customer service.

  • Participate in team meetings, training, and quality improvement initiatives.

  • Perform additional administrative duties as assigned.

    Knowledge & Skills
    • Knowledge of home care regulations and compliance requirements is preferred.

    • Familiarity with state regulations, Medicaid waiver programs, and Veterans Affairs (VA) requirements is highly desirable.

    • Understanding of caregiver credentialing, documentation standards, and regulatory compliance.

Qualifications
