Job description

Job Title: Qualified Professional (QP)
Department: Intellectual/Developmental Disabilities Ministry
Reports To: Director of Intellectual/Developmental Disabilities
Revised Date: March 2020

SUMMARY
The QP oversees the assigned home(s) and enables the team, residents and families to function at their highest level of competency. This position also ensures that the agency’s policies, procedures and programs are delivered appropriately and that the home remains in compliance with state regulations.
QUALIFICATIONS:
• Meets the general qualifications for employment as outlined in the Institution’s personnel policies.
• Must be at least 23 years of age.
• Must meet requirements for QP - be a college or university graduate with
1. a master’s degree in related human service field and one year of full time, pre- or post-graduate accumulated experience with the population served, or
2. a baccalaureate degree in a related human service field and two years of full time, pre- or post-graduate accumulated experience with the population served, or
3. a baccalaureate degree in a field not related to human services and have four years of full time, pre-or post-graduate accumulated experience with the population served.
• Have at least 2 years of successful work experience in the Developmental disabilities field which will include supervisory or administrative responsibilities.
• Direct care experience and/or group home management experience preferred.
• Education, experience, skills and abilities compatible with the responsibilities outlined in this job description.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Supervision:
• Works under the direct supervision of the Director of IDDM.
• Participates in 90-day and bi-annual performance evaluations.
• Prepares for and participates in supervisory conferences with the Director of IDDM.
• Maintains an open line of communication with all staff, residents and families.
• Participates in staff training opportunities provided by the Institution and any other agencies as requested by the supervisor, and/or required by the State of North Carolina.
General Responsibilities:
• Manifests in daily action a Christian spirit and lifestyle by incorporating Baptist Children’s Homes (BCH’s) core values and philosophy.
• Demonstrates a working knowledge and understanding of
1. the Institution’s structure, services, policies and procedures;
2. job performance requirements;
3. related jobs.
• Respects the cultural and socioeconomic diversity of staff members and residents in care and strives to create a positive relationship within BCH.
Clinical Responsibilities:
• Provides intake services for prospective residents and their families.
• Participates in all admission and discharge planning affecting the residents residing in the group homes in their area of responsibility.
• Completes all assessments and required documentation for new admissions.
• Develops Person Centered Plan (PCP) for each resident in conjunction with that resident’s treatment team.
• Evaluates goal progress and completes monthly progress note for each client and makes part of client record and sends a copy to the guardian.
• Manages Personal Care Services (PCS) applications and assessments and oversees provision of PCS according to the plan of care.
• Schedules and participates in team meetings as needed for treatment.
• Reviews and completes incident reports and initiates required action. Completes and submits quarterly incident report to (Managed Care Organization) MCO.
• Reviews resident records and assures accurate and thorough documentation.
• Ensures compliance with regulations governing group homes licensed under the Division of Health Service Regulation (DHSR).
• Participates in DHSR surveys, MCO reviews and any other external monitoring.
• Completes Performance Quality Improvement (PQI) reports on a quarterly basis.
• Develops and maintains a client rights committee.
• Develops a supervisory plan and provides monthly supervision to direct support staff and maintains documentation of supervision.
• Provides initial and annual required clinical trainings to Direct Support Professionals (DSP), Innovations Support Professionals (ISP), and Administrative- Assistants, as relevant.
• Provides clinical oversight and supervision for Supervised Living services, Innovations Residential Supports, Innovations Home and Community Supports.
• Provides clinical supervision to ensure acquisition, improvement and retention of skills related to activities of daily living and social and adaptive skills; to ensure that interactions with the resident are designed to achieve outcomes identified in the plan of care; to ensure that habilitation, training and instruction are coupled with elements of support, supervision and engaging participation to reflect the natural flow of training, practice of skills, and other activities as they occur during the person’s day; to ensure provision of treatment interventions to ensure that the resident acquires skills necessary to compensate for or remediate functional problems as outlined in the person-centered plan; to ensure that support and supervision in a home environment will enable the resident to participate in a supportive, therapeutic relationship where the primary purpose of care, habilitation, or rehabilitation is provided.
• Reviews documentation related to clinical services in resident records and assures accurate and thorough documentation, to include medication administration.
• Attends and participates in quarterly meetings to include peer review, compliance updates, administrative updates, staff enrichment, and PQI committee.
• Conducts Quarterly Clients Rights Committee, Monthly Resident Council Meetings and Quarterly Parent/Guardian Meetings.
Administrative Responsibilities:
• Oversees administrative duties regarding payroll, personnel, financial and budgetary, and quality improvement.
• Reconciles residents’ personal funds monthly and sends statement to parents/guardians on a quarterly basis.
• Participates in hiring qualified staff and ensures that all duties are carried out in a timely and appropriate manner.
• Supervises and schedules staff for proper coverage.
• Completes 90 day evaluation for staff under his/her supervision.
• Develops a safety committee for the group home and ensures that safety standards are being met. Ensures safety committee meets quarterly and minutes are completed and submitted. Ensures monthly facility inspection checklist is completed and submitted.
• Develops and maintains emergency response plans for a variety of possible scenarios and trains staff/residents on procedures. Ensures emergency supplies are maintained and current in the home(s).
• Completes Month End Status Report.
• Assist to complete needed paperwork to ensure continuation of Social Security and other financial benefits for residents.
• Ensures disciplinary documentation is properly recorded and in compliance with EEOC laws and participates in disciplinary action/termination of personnel, as needed.
• Completes Performance Quality Improvement and other reporting documentation on a quarterly basis.
• Develops a marketing plan to increase awareness of BCH/IDDM.
• Maintains a current and accurate business contact listing and develops those relationships (through visits, phone calls, letters, vacancy listing) to advertise BCH and IDDM vacancies.
• Works closely with Director and development and communications department to integrate stories and photos to website and other publications as well as keep the Needs List current and available.
• Speaks in churches and various other organizations to promote IDDM and BCH as requested.
• Participates in Council on Accreditation as requested.
• Responsible for the Homes Budget and overall expenditures
• Responsible for keeping 100% of beds at full capacity to meet budget
• Monitors Payroll for accuracy and timely submissions
• Reviews Electronic Records for accuracy and timely submissions
PRN Direct Care Coverage:
• When QP cannot secure coverage for the group home(s), QP will serve as PRN Direct Service Professional (DSP) and will fulfill the duties of that position as follows:
Essential Duties and Responsibilities:
• Provides support and supervision in a home environment to enable the residents to participate in community activities, social interactions in the home, and participate in a supportive, therapeutic relationship where the primary purpose of the service is care, habilitation, or rehabilitation.
• Provides treatment interventions to ensure that the residents acquire skills necessary to compensate for or remediate functional problems as outlines in the person centered plan.
• Provides supervision, both on an individual and group basis, oversees residents’ daily activities and personal care, assisting and supervising as needed.
• Provides Christian role model and spiritual guidance and a Christian environment to include devotions and regular church attendance.
• Prepares and provides well balanced meals as outlined by the agency’s menus and daily nutritional requirements of residents.
• Establishes structure in the home to enable the residents to learn responsibilities and to become functioning members of the home.
• Generates an atmosphere which helps each resident feel accepted and respected as a group member.
• Maintains a safe home environment including instructing group in safety, fire prevention, fire drills and evacuation plans per policy and procedure.
• Plans events for residents, including informal social time, special events and group activities as outlined by the QP, maintaining the required weekly amount of 14 hours.
• Complies with safety in accordance with OSHA, food program, sanitation, building code, accreditation standards, and N.C. licensing requirements.
• Provides transportation for residents to events, including but not limited to school, work, church, and medical appointments.
• Purchases food, supplies, and equipment to meet the needs of each resident according to schedule maintained in group home and upon approval by the QP.
• Greets visitors in a positive and hospitable manner.
• Provides attention to the medical and dental health needs of each resident as outlined in policies and procedures and directed by the QP.
• Maintains agency vehicle in good operating condition by ensuring regular maintenance service. Report any repairs needed to the QP.
• Teaches weekly life skills curriculum to residents and allows for group participation and learning.
• Meets the general qualifications for employment as outlined in the Institution’s personnel policies.
Professionalism/Teamwork:
• Represents the institution to various individuals, groups, and public in a professional manner.
• Maintains professional standards as set by the institution.
• Respects the rights of the total team and keeps other team members informed of any changes in the home by developing a way to disseminate timely information to all staff.
• Respects the cultural and socioeconomic diversity of staff members and residents and strives to create a positive relationship within BCH.
• Participates in the agency’s Performance Quality Improvement (PQI) program as needed.
Policies and Procedures:
• Maintains all handbooks and support materials.
• Completes all paperwork necessary to document services provided and as required by licensure, regulations, and the institution to include, but not be limited to: goal tracking, personal care, resident log, community outings, therapeutic leave, contact notes, service notes, sign in/out form, MARs and all medication and medical services related paperwork, changeover forms, vehicle mileage, incident reports, fire and disaster drill documentation including extinguisher and emergency lights, resident cash and checking documentation, petty cash documentation, filing of documentation in appropriate locations; and other paperwork as needed by the agency or assigned by supervisor.
• Maintain proper administration of ALL medications, medication storage, and medication records for each resident.
• Participates in all required in-service training.
• Assures all rights listed in the Domiciliary Home Resident’s Bill of Rights are exercised for each resident and gives assistance to residents in exercising these rights.
• Responds to grievances according to the Institution’s policies and procedures.
• Maintains confidentiality regarding each resident and his or her records.
EQUIPMENT:
Ability to operate standard office machines such as copier, computer, fax, calculator, etc. Ability to work within Google, Outlook, Excel, and Word software programs. Must be able to drive a 12-passenger van and get in and out of the van unassisted.
LANGUAGE SKILLS:
Ability to read, analyze, and interpret general information. Ability to effectively communicate with others, including managers, clients, customers, and the public.
CONTACT WITH OTHERS:
Regular contact with colleagues in all departments, as well as with outside individuals and groups; should represent Baptist Children's Homes of NC, Inc. in a positive manner. Contacts outside of the agency where proper handling of situations is of utmost importance as the agency’s image could be affected.
MATHEMATICAL SKILLS:
