Job description

The MCHBG Action Plan Coordinator (PHPD 2) provides leadership and coordination for Maternal and Child Health Block Grant (MCHBG) strategic planning, implementation, and performance reporting activities. This position supports the Division's efforts to achieve measurable improvements in maternal and child health outcomes by ensuring effective planning processes, monitoring progress toward established goals, and facilitating collaboration across programs and priority areas. Working closely with program leadership as well as internal and external partners, the MCHBG Action Plan Coordinator promotes alignment between program activities, federal requirements, and quality improvement principles.

The position serves as a central resource for developing action plans, monitoring performance, reporting, and continuous improvement efforts.
