• Perform comprehensive mental health assessments, considering the interplay between mental and physical health challenges that affect our patients.
• Create and implement individualized treatment plans that address both psychiatric and medical issues to promote optimal recovery and well-being.
• Facilitate individual and group therapy sessions where evidence-based therapeutic techniques are used to help patients navigate their challenges.
• Collaborate closely with medical professionals to ensure seamless integration of care, discussing treatment options and patient progress during team meetings.
• Provide crisis intervention during mental health emergencies, offering immediate support and stabilization to at-risk patients.
• Educate patients about the relationship between mental health and physical wellness, empowering them to take charge of managing their mental health conditions.
• Ensure meticulous documentation of patient records in compliance with legal and ethical standards.
• Conduct regular participation in team meetings and case conferences to develop cohesive treatment strategies as part of an interdisciplinary team.
• Stay informed about current research and best practices in integrated care, attending continuing education opportunities as needed.

You will work primarily with veterans and their families who may be experiencing complex mental health needs due to co-occurring medical conditions. Our patients often face challenges such as PTSD, anxiety, depression, and trauma-related issues, requiring specialized interventions and a compassionate approach to care.

• Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current licensure as an LCSW, LPCC, or LMFT in the state of California is required.
• Demonstrable experience in managing mental health care for individuals with co-occurring medical conditions.
• Strong interpersonal and communication skills that foster a therapeutic client relationship.
• A commitment to maintaining professional boundaries and adhering to ethical guidelines while ensuring patient confidentiality.

• Familiarity with electronic health record (EHR) software and other clinical applications.
• Experience working collaboratively in an interdisciplinary team environment.
