Apply today to join the PSG team!JOB SUMMARY:PSG is a community-based social services organization looking for dedicated mental health and social services professionals like you to join our CCS Service Facilitation program in Jefferson County.The Service Facilitator provides intensive case management services to children and adults who have a severe emotional disturbance (SED). The CCS Service Facilitation program follows the wraparound model of service delivery to identify needed services and develop multi-disciplinary treatment plans that address both the strengths and needs of the family or individual. As a Service Facilitator, you will collaborate with case managers, family advocates, mental health providers, AODA providers, school support staff, juvenile justice workers, child welfare workers, and other community providers to ensure our clients receive the best possible care.
