Job description

Position Description:

The Community Support Coordinator coordinates and provides support to people receiving services from Options and works as part of a team primarily supporting individuals on the autism spectrum. Each Community Support Coordinator has a small caseload and maintains a flexible work schedule based on the needs of individuals they supportย Mon-Fri 9am to 5pm. Occasional evening and weekend hours required. Applicants must have an insured vehicle and a good driving record. Due to on-call responsibilities, applicants must live with-in 30 minutes of Madison or be willing to relocate.

Duties and Responsibilities:

  • Develop and implement individualized support plans including behavioral support plans
  • Hire, train, and supervise direct support staff
  • Provide direct support and personal care, as needed
  • Convene and run regular household meetings
  • Assist people supported to manage personal finances
  • Maintain accurate and timely documentation of time spent and log notes
  • Coordination of medical care
  • Work cooperatively with family, friends and other service providers

Candidate Requirements:
