Job description

Make a difference in the lives of New York City's older adults, their families and the community. For over 60 years PSS has performed vital and important work - through its 10 senior centers, two apartment buildings, caregiver services and affiliated programs, the agency touches the lives of over 2,000 people a day. As anOutreach Care Managerfor the PSS Circle of Care program the goal is to generate client referrals by cultivating community partners and engaging potential caregivers in the community. The Outreach Care Manager's role is vital in raising the profile of PSS Circle of Care within the community. This position requires extensive fieldwork in the community - primarily Bronx and Manhattan.

This position reports to the Director in the Circle of Care Program

RESPONSIBILITIES:

  • Meet grant outreach expectations.
  • Conduct street level outreach in areas frequented by target population in the Bronx & Manhattan
  • Strategize to promote the program and its services by cultivating new partnerships (community agencies, nonprofits, faith-based institutions and for-profit entities) to generate caregiver referrals.
  • Build relationships with Community Leaders and Boards within Community Districts and stakeholders.
  • Conduct regular field work to potential organization partners etc.
  • This position will require the candidate to keep a robust calendar of outreach leads, meetings and outreach events.
  • Outreach to potential caregivers over the phone, email/ mail, and in the community.
  • Participants in program initiative for outreach purposes
  • Communicate regularly with referral partners on a set schedule.
  • Identify content partners to bring new and innovative programming to PSS Circle of Care.
  • Attend and participate in monthly PSS Circle of Care Team Meetings
  • Schedule and facilitate, presentations, in the community to raise awareness about PSS Circle of Care
  • Promote PSS Circle of Care programming and services on Social Media (Instagram, Facebook).
  • Represent PSS in the media, at community fairs and events (table at community health fairs, hospitals, events and other localities)
  • Organize and host community health fairs
  • Log referral partners and track units of service by maintaining an accurate Worker Log and maintaining Caregiver database entries in VIVE.
  • Provide support to Care Managers as needed.
  • Other duties assisted by supervisor

QUALIFICATIONS AND EXPERIENCE:

  • Bachelor's degree in relevant field
  • Social Work, Public Speaking, sales and marketing experience preferred
  • Excellent Interpersonal and Organizational Skills
  • Excellent Office Management and Computer Skills with proficiency in Microsoft Office
  • Excellent communication skills (written and oral)
  • Bilingual fluency (English/Spanish) desirable
