Job description

The Patient Services Coordinator/Scheduler (PSC) plays a vital role in coordinating patient care by managing visit scheduling and ensuring adherence to ordered visit frequencies for home health services. Working onsite in the office, this position is responsible for monitoring physician-ordered visit frequencies, coordinating visit dates and times, and maintaining accurate scheduling to support continuity of care.

The PSC collaborates closely with patients, caregivers, and clinical staff to ensure visits are scheduled efficiently and in alignment with the patient's plan of care. Under the supervision of the VP of Home Health Operations and Executive Director/Clinical Manager, this role helps ensure timely service delivery, effective communication, and optimal coordination of the care team to meet patient needs.

LPN license requried and in good standing
