Psychiatric Rehabilitation Service Coordinator
(casemanager)
Definition
The Psychiatric Rehabilitation Service Coordinator (PRSC) is responsible for
coordinating the overall delivery of care to each individual within his/her caseload. The
incumbent functions as a “case manager.” The PRSC must be knowledgeable regarding
federal and state regulations which address the care of the mentally ill adults in a longterm
care facility. The PRSC is responsible for helping individuals address their mental
health challenges enabling each to function at his/her highest practical level

This
includes duties pursuant to OBRA F406 and Illinois Department of Public Health-
Subpart S.
Delegation of Authority
As the PRSC, you are delegated the administrative authority, responsibility and
accountability necessary to carry out your assigned duties. You are accountable
both administratively and clinically to your immediate supervisor. Additional,
clinical supervision will be provided by the facility’s MSW consultant.
Qualifications
1

Bachelors or Master’s degree in a human service profession (i.e., Social Work,
Psychology, Sociology, Counseling Psychology) and one (1) year of supervised
experience in mental health or human services.
2. Demonstrated skills and ability in working with and understanding the needs of the
residents, families, staff members and personnel from community agencies.
3. Demonstrated sensitivity to the needs of the mentally ill population.

Demonstrated familiarity and knowledge of current trends in the rehabilitation of
the mentally ill.
5. The ability to present concise, meaningful written reports and articulate the
social/psychological needs of the residents.
6. The ability to accept and utilize professional supervision, consultation and inservice
training.

The ability to handle confidential data with professional discretion.
Responsibilities
1. The responsibilities of the Psychiatric Rehabilitation Service Coordinator are:
A. To provide the resident with a stable therapeutic relationship

To orient the resident to the facility
C. To review and assist the resident in understanding the treatment plan and
program schedule
D. To prepare and assist the resident with active participation in the treatment

To provide and/or coordinate the delivery of the psychiatric rehabilitation
services programs, and
F. To monitor the resident in the areas of self-directed care and for overall
compliance with the treatment plan
Psychiatric Rehabilitation Service Coordinator duties include:
2. Welcoming the new resident, providing support, establishing a trusting relationship
and initiating the assessment process.

Completing assigned sections of the MDS, assigned RAPs and additional
components of the resident’s comprehensive assessment, per facility policy. Some of
these assessments may include the Psychosocial History and Assessment, skills
assessment, discharge plan, and assessment of resident interests and expectations.
4. Re-evaluating the resident on a quarterly basis and documenting this re-evaluation.

Overseeing the interdisciplinary team (IDT) in the development of the individual
treatment plan (ITP). The ITP should address the reduction of the resident’s
psychiatric symptoms and the acquisition of skills necessary to enable the
individual to achieve a higher functional level.
6. Documenting the resident’s progress on a regular basis, including incidental and
quarterly notes

Charting may occur more frequently per facility protocol. The
documentation should focus on: (a) successfully completed objectives, (b) skill
regression, (c) areas in which progress is limited and what reasonable efforts have
been attempted to facilitate advancement, and (d) new objectives.
8. Providing “active listening” with individuals on the assigned caseload

The PRSC
needs to establish a good rapport with residents and be available to provide
behavioral intervention/counseling.
9. Contacting community agencies for the purpose of resident referral. These agencies
include, but are not limited to: the Department of Mental Health, Department of
Public Aid, Department of Rehabilitation Services, Veteran’s Administration,
community mental health centers, substance abuse programs, workshops,
vocational programs, hospital-based programs, social organizations, etc.

Working with the licensed [clinical] social work consultant and program
supervisors to: (a) develop and implement essential in-service training programs;
(b) develop program policies and procedures, job descriptions and (c) evaluate
program outcomes and recommend program changes, as needed.
11. Maintaining knowledge of residents on psychotropic medications, including the

identified purpose of a specific medication and participating in behavioral
programming to help the resident achieve his/her highest functional level.
12. Providing direction and supervision to assigned staff.

Leading group and individual counseling sessions, as assigned.
14. Assisting the resident to participate in individual and group programs and to utilize
these programs to full advantage. Including encouraging each resident to do as
much for him/herself as possible.

Meeting with the program consultants/supervisors for educational purposes,
direction and performance appraisal/evaluation on a regularly scheduled basis.
16. Participating in the “on-call” schedule and being available to work flexible hours
(i.e., evening and weekend coverage).
17. Being familiar with the facility policy on Behavioral Emergencies and be trained in
crisis management and diffusing a potentially volatile situation (especially
concerning aggressive/agitated behavior).
