Job description

Position Summary:

The PRSC plays a vital role in the care of residents with mental health diagnoses by developing and monitoring individualized psychiatric rehabilitation plans, coordinating with behavioral health providers, and ensuring regulatory compliance with state and federal guidelines. This role bridges clinical care with social and emotional support to help residents achieve optimal functioning and quality of life.

Key Responsibilities:

  • Assess the psychosocial and psychiatric rehabilitation needs of residents upon admission and regularly thereafter

  • Develop, implement, and monitor Individualized Recovery or Rehabilitation Plans (IRPs) in coordination with the interdisciplinary team

  • Coordinate services with psychologists, psychiatrists, case managers, social workers, and direct care staff

  • Facilitate or oversee therapeutic groups and mental health programming

  • Ensure documentation complies with CMS/state psychiatric rehab regulations (e.g., MDS Section Q, PASRR, treatment goals)

  • Advocate for resident rights and dignity, especially regarding psychiatric and behavioral health needs

  • Serve as liaison with managed care organizations, community mental health services, and state agencies

  • Participate in discharge planning and community reintegration efforts when applicable

Qualifications:

  • Required: Bachelor’s degree in psychology, social work, counseling, or related field

  • 1+ year of experience working in a behavioral health or psychiatric setting, preferably in long-term care or SNF

  • Familiarity with state psychiatric rehabilitation regulations, PASRR Level II documentation, and individualized treatment planning

  • Strong communication, advocacy, and organizational skills

  • Empathy, cultural competence, and ability to work with diverse resident populations
