Primary Health Care (PHC) was founded in 1981 by Dr. Bery Engebretsen in Des Moines, IA. Our mission has remained unchanged since that time, to provide healthcare and supportive services to all, regardless of insurance, immigration status, or ability to pay.

Based on the needs of the communities we serve, PHC offers a spectrum of medical and dental services including family practice, behavioral health, HIV care and services, and pharmacy. PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown.



As a Health Care Navigator, you will function as a member of the Supportive Services for Veteran Families (SSVF) team and provides services that include connecting Veterans to health care benefits, case management, care coordination, health education and interdisciplinary collaboration. The Health Care Navigator works closely with the Veteran’s assigned multidisciplinary team, including medical, nursing, administrative specialists and case management personnel to ensure each Veteran is provided with timely, appropriate, and equitable care.

What You Will Do

  1. Conducts non-clinical assessments with Veterans and interdisciplinary team to identify and address potential barriers to care; creates a Health Care and Housing Stability Plan.
  2. Identifies appropriate referrals for Veterans which include but not limited to, housing financial benefits, and transportation.
  3. Monitors and documents client contacts, needs, goals, progress, and significant occurrences, according to grant guidelines and PHC policies and procedures.
  4. Works collaboratively with SSVF team and other community providers to ensure Veteran centered care; identifies and addresses system challenges for enhanced care coordination.
  5. Identifies the health education needs of the household and provides education services and materials that meet the literacy level of the household.
  6. Communicates with other agencies on behalf of clients using professional discretion regarding client information; ensures appropriate releases of information are completed.
  7. Builds rapport with Veterans and ensures preferences are honored by assisting in communicating with healthcare team.
  8. Participates in development of the Veteran’s Housing Stability Plan in collaboration with Veteran and interdisciplinary team.
  9. Collaborates with community providers to maximize the use of VA and community resources.
  10. Ensures clients have access to transportation for appointments and assists with client transportation needs in accordance with the PHC transportation policy as appropriate.
  11. Conducts outreach activities at community agencies, community gatherings, neighborhoods, shelters, and other appropriate venues as requested. Serves as liaison between PHC and community resources.
  12. Performs other duties as assigned.

Your experience and skills determine your base pay. The hiring range for this position is typically $17.00 - $21.25 per hour. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $25.50 per hour.
